Evaluate …Implement…Document… and Plan Trach Care

The George Washington University Hospital has a Physician Order Set to be utilized for all patients with a Trach. This pre-printed order set is to be transcribed onto the Tracheostomy Procedure Documentation Form.

Tracheostomy Orders : Physician Order Sheet

The physician adds the date and time to the order sheet. The physician identifies the trach type, size and date placed.

There is a list of pre-printed orders, which can be customized by the physician.

The order set is signed by the physician and placed in the chart with the other physician orders.

Any staff trained to transcribe orders may transcribe the orders onto the

‘Tracheostomy Procedure Documentation Form' . The nurse must sign the order form indicating that the orders were transcribed correctly.
